Job Summary:

Speech-language pathologists (SLPs) work to prevent, assess, diagnose, and treat speech, language, social communication, cognitive-communication, and swallowing disorders in a senior care environment.
Responsibilities and Duties:
- Screens and evaluates patients assigned per MD orders and as indicated, and establishes appropriate Plan of Care and goals that meet the specific needs of the individual.
- Diagnoses patients and develops treatment plans for swallowing, speech, language, fluency, or voice impairments.
- Develops individual and/or group activities and/or programs to manage speech, language or swallowing problems.
- Provides training and education to staff, family, and caregivers as indicated.
- Observes and records patient's progress, response to treatment and limitations and maintains this information in electronic medical record.
- Completes required documentation at point of service.
- Reports to Director of Rehabilitation/designee on patients' progress and discharge recommendations.
- Attends and reports accurately a patient's status at weekly UR meeting and interdisciplinary care plan meeting.
- Screens all residents in facility on a quarterly basis, to identify those who have experienced declines in their functional abilities and determine if Speech Therapy intervention is indicated.
- Re-assesses patients and completes Updated Plan of Care as indicated on initial evaluation, in order to determine need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated.
- Identifies needs for program development and implement programs according to company policies and procedures.
- Coordinates in-services for facility staff as need is determined, to ensure safe and effective carry over for patient care.
Qualifications:
- Education: Graduate of an accredited therapy program
- Licenses/Certifications: Current Speech Therapist New Jersey State license
- ASHA certification

Salary : $90,000 - $120,000
