Hospice Intake Coordinator, Full-Time

JHC Hospice is seeking a compassionate, organized Intake Coordinator to help patients, families, and referral partners navigate the transition to hospice care. Job Type: Full-TimeSchedule: M-F 8:30am to 5pmPay Rate: $30 - $38 per hour depending on experience

Responsibilities

• Receive, review, and process hospice referrals from hospitals, physician offices, nursing facilities, and community sources.
• Gather and verify required admission information, including medical records, physician orders, insurance coverage, and demographic data.
• Serve as a primary point of contact for patients, families, and referral sources throughout the referral and admission process.
• Coordinate admission assessments and visits with hospice leadership and clinical staff to ensure timely patient access to services.
• Track referral activity, monitor outstanding items, and follow up to facilitate efficient admissions.
• Maintain complete and accurate referral records in accordance with agency standards.
• Develop and sustain positive working relationships with healthcare providers and community referral partners.
• Support smooth communication between referral sources and hospice team members to ensure continuity of care.
• Adhere to agency policies, regulatory requirements, and confidentiality standards in all aspects of work.

Requirements:

• Background in nursing, social work, healthcare support, or a related healthcare field.
• Experience in hospice, intake, care coordination, case management, or patient services preferred.
• Compassionate communicator with the ability to navigate sensitive conversations with empathy and professionalism.
• Strong organizational, multitasking, and problem-solving skills in a fast-paced environment.
• Proficiency with EMR systems and standard computer applications.
• Ability to prioritize urgent needs, manage competing demands, and meet deadlines.
• Respect for diverse cultural, spiritual, and personal beliefs surrounding end-of-life care.
• Self-motivated team player who can work independently and collaborate with a multidisciplinary team.
