Position Summary: The Psychiatric Case Manager will have a caseload and meet with clients, assess needs, and coordinate services. The Case Manager works with clients to strengthen family functioning. He/she develops treatment plans with client’s participation to address mental health, social, medical and material issues.

The Case Manager’s caseload will include survivors of domestic violence and sexual abuse. The incumbent supports survivors of domestic violence in locating and accessing safe and affordable housing as well as legal consultation and representation as needed.

The Psychiatric Case Manager plays a crucial role in ensuring the smooth functioning of the agency’s psychiatric practice. This position provides high-level administrative support primarily to the psychiatric providers as well as the team of therapists, and other mental health professionals while maintaining a welcoming, professional environment for clients. Responsibilities include scheduling, client communication, insurance coordination, documentation, and supporting clinical operations. The ideal candidate is detail-oriented, organized, calm, and able to handle sensitive information with discretion and empathy.

Main Responsibilities:

· Conduct initial assessments, and then develop treatment plan with client’s participation

· Advocate on client’s behalf to secure safe and stable housing

· Help families access resources for housing related assistance (i.e., furniture procurement, utility assistance, etc.).

· Provide advocacy, safety planning, resources, and support to survivors of domestic violence

· Accompany clients to appointment, visit clients at home, and provide transportation to/from the Agency – as needed. (Agency vehicle will be provided when driving a passenger.)

Client Support & Communication

  • Schedule new client intakes, medication management appointments, follow-ups, and rescheduling as needed.
  • Act as a liaison between clients and psychiatric prescribers for non-clinical matters and treatment-related questions.
  • Assist clients with completing intake paperwork and navigating telehealth platforms for virtual appointments.
  • Contact clients for appointment reminders, follow-ups, and medication authorization updates.

Administrative Tasks and Team Support

  • Maintain accurate and up-to-date client records in both paper and EHR (Electronic Health Record) systems.
  • Assist with prior authorizations and communicate with insurance companies to confirm coverage and benefits.
  • Monitor psychiatric schedules and manage provider inboxes as required.
  • Coordinate with billing and other administrative staff to ensure smooth workflow and timely completion of tasks.

Other

- Actively contributes to a positive team environment and provides the collaboration needed for high-quality services to survivors.

- Participates in training, coaching, and learning communities to increase knowledge of housing and support for survivors.

- Participates in data collection and reporting efforts as required by the program and funders.

- Assists in the coordination and marketing of program activities.

- Keeps statistical records, tracks use of resources and documents encounters.

- Participates in supervision and brings attention to any circumstances that present significant risk or harm.

Qualifications:

  • Valid Driver License.
  • Spanish/English, Bi/multilingualism
  • Knowledge about domestic violence and the impact of violence and trauma on survivors and their children.
  • Knowledge of housing resources and options, public benefits, and community resources to meet various survivor needs.
  • Proficiency with scheduling software, EHR systems, Microsoft Office Suite, and telehealth platforms preferred.
  • Demonstrated history of contributing to a healthy and collaborative team culture.
  • Ability to work in a multi-task environment and accept interruptions while performing duties.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal and conflict resolution skills.
  • Excellent organizational skills and attention to detail.
  • Strong analytical and problem-solving skills.

Education: Bachelor’s degree minimum

Pay: $24.11 - $30.03 per hour
