Program Description

In an environment with a rapidly aging population, JFCS East Bay’s highly skilled older adult services team members provide personalized aging consultation, assessment, and care management for older adults and their families of all backgrounds, religions, and income levels. We support local East Bay Holocaust survivors and other members of the Jewish community experiencing challenges. As our services continue to grow in demand, our staff work collaboratively to partner with clients to support them in meeting their immediate needs and long-term goals.

Position Summary

The geriatric care manager provides consultation and direct care management services with older adults and Holocaust survivors and their families in Alameda and Contra Costa counties. This position supports the Jewish Community Safety Net which helps to stabilize Jewish community members in the East Bay who are going through challenging times. The geriatric care manager will join a collaborative team of thoughtful and dedicated social service professionals with shared responsibilities of facilitating a range of support groups and supportive social activities, resource navigation, providing individual psychotherapy, and partnership management.

The geriatric social worker works under the supervision of the Director of Adult Services.

Position Responsibilities

Care Management

  • Provide short-term consultation services to older adults and their families.
  • Provide ongoing care management services to older adults and those with disabling diagnoses including MS. Conduct assessments and develop care plans for clients, that reflect client preferences and values; in selected cases, implement and monitor care plans.
  • Respond to the needs and challenges of home care clients, conducting home visits as required.
  • Provide social services and financial assistance disbursement to low-income Jews in the community through the Jewish Community Safety Net Program.
  • Provide information and referral services to older adults and family members.
  • Support volunteers working with older adult clients.
  • Provide trauma informed care/social services to Holocaust Survivors.

Project Management

  • Support the administration of the Jewish Community Safety Net Program
  • Assist in outreach and promotion of JFCS East Bay elder care services.

Support Groups and Counseling

  • A range of support groups and individual short- and long-term psychotherapy services are offered by this team.

Opportunities for providing mental health services are dependent on the professional qualifications of candidates and may not be included in the role at the start of the position.

Required Qualifications

  • Geriatric social work knowledge, skills, and experience.
  • Ability to communicate effectively with older adult clients, families, and other community representatives.
  • Project management skills
  • Demonstrated capacity to be open-minded and solution focused.
  • Cultural competence in working with people of various nationalities, ethnicities, and religious affiliations.
  • Superior communication and interpersonal skills while ensuring confidentiality.
  • Excellent record-keeping and documentation skills.
  • Competent computer skills including Microsoft Word and Excel; ability to learn client tracking system.
  • Master’s Degree from an accredited university or college in social work, counseling, rehabilitation, gerontology, or related field.

  • MSW, ASW or LCSW (JFCS East Bay is able to provide clinical supervision for ASWs completing their clinical hours needed for licensure.)

Compensation and Benefits

The compensation for this position will range from $75,000 - $85,000 based on experience.
