ELECT is a cooperative venture designed to improve educational opportunities and social service supports for pregnant and parenting teens. The goals of the program are to promote high school graduation and reduce repeat teen pregnancies. Case Managers will be responsible for direct support and education with students involved in the program. The Case Manager will be assigned to specific schools and community sites throughout the city of Philadelphia. Case Managers will be responsible for direct support and education for students enrolled in ELECT.

- Recruit and Retain students for the program
- Create welcoming, safe and trauma informed environment in each assigned site
- Maintain well organized ELECT sites
- Responsible for working with students individually and building ongoing relationships with them and their families.
- Perform assessments of each student's strengths and needs and creating a unique Motivation Interviews for each student.
- Monitor students' fulfillment of ELECT benchmarks
- Fulfill required monthly service hours (4) and quarterly home visits with every assigned student.
- Facilitate workshops every other week and assess students' understanding of the concepts presented.
- Visit students in their homes when the need arises such as Teen Parent Leave, but no less than quarterly.
- Participate in monthly Saturday events with ELECT students and their families.
- Provide recognition opportunities and monthly incentives to students.
- Partners with families and school personnel to ensure that all needed services identified are fulfilled.
- Perform daily, accurate and complete data entry (attendance, case notes and all required forms) on a timely basis with quality checks performed by Team Leader and Data Coordinator.
- Maintain program records and files, compile data and prepare reports as needed to comply with funding requirements promptly and efficiently.
- Establish a positive working relationship with all school and community site personnel and be knowledgeable about resources within your school or community sites.
- Schedule and attend monthly team meetings with Points of Contacts at each site.
- Attend staff meetings and supervisory sessions.
- Maintain daily communication by checking e-mail and phone messages at least twice per day.
- Submit monthly expense forms for mileage and other accrued expenses.
- Actively link students to school and community resources as needed.
- Adhere to EOS procedure for recording daily attendance and by weekly time sheet.
- Assist in the development and facilitation of the EOS strategic plan.
- Maintain awareness of JFCS vision, mission and program components.
- Seek and take advantage of opportunities for professional growth and attend mandatory trainings and meetings.
- Perform other duties as assigned by immediate team leader and other management as requested.
- Knowledge of prenatal and child development.
- Knowledge of trauma informed care.
- Proficient use of technology (smart phone/tablet/laptop) and experience using MS Office applications is required.
- Ability to exercise independent judgment in an area of activity for which procedures cannot be standardized.

Education And Experience Requirements
- B.S.W. or B.A. in Human Services or related field;
- Experience working with youth in an urban setting.
- Prior experience working with pregnant or parenting teens is preferred
- One to three years' experience in a children's services program environment highly preferred.
- One to three years prior experience facilitating educational workshops preferred.
- Due to the movement of presentation materials from office to work locations, the employee must be able to lift up to 30 pounds and be able to move materials from their vehicle into the work location.
- All prospective employees must pass a criminal background check, child abuse check and FBI fingerprint clearance.
- Must own a car, have and maintain a safe driving record on file with the Department of Motor Vehicles in the state in which the employee is licensed to drive. Must provide a valid driver's license, maintain proof of automobile liability insurance and must maintain coverage.
- Due to reduction in summer service hour requirements, ELECT Case Manager may be assigned additional direct service responsibilities in the Summer Work-Ready Program, Parent- Child Collaborative Program or other programming as needed
- Some occasional evening work may be required
- Employee must follow current vaccination requirements of the agency
