What are the responsibilities and job description for the Community Coordinator of Integrated Maternal Health & Wellness position at Jewel of Justice?
The Position This position is made possible by our 18-month second round of funding grant (September 15, 2026 – March 15, 2027) from the Packard Foundation which focuses on Advancing Black Maternal Behavioral Health. We are looking for an individual who has a demonstrated track record of working with and for Black and African American communities, and who is ready to share their creative energy, knowledge, and skill-set towards addressing Black maternal and infant mortality in Fresno County.
Position Responsibilities This person will report directly to the President and Project Manager, Dr. Karen Crozier, and engage the Leadership Council (LC) 4 Black Self-Care & Wellness, a 15-member body that provides leadership, support, and accountability in the Advancing Black Maternal Self-Care & Wellness Series: An Eco-Village Approach. Currently, the Series has seven core sessions a year that runs from Black History Month through Juneteenth, and at least one bonus outdoor, 3 days and 2 nights, camping session from July through August.
The responsibilities include, but are not limited to, the following:
1. Establish, maintain, or access relationships with diverse health care providers, primary care physicians, health and wellness related community-based organizations, and managed care providers for program promotion and referrals.
2. Implement, and modify as needed, intake process of Series, and prospective Series, participants who desire to work more extensively with Jewel of Justice in providing them support to access desired services and support.
3. Be able to manage a case load of up to five (5) clients per week that includes intake assessments, reminder e-mails or phone calls, follow-up phone call or emails, and/or individual or small group sessions.
4. Assist in providing resources and support services for Series participants in 1:1 or small group managed care settings online, by phone, home visitation, satellite office, and/or community venue.
5. Attend all Series sessions.
6. Coordinate promotion and distribution of materials at sessions for company and partners.
7. Attend staff meetings with Project Manager and Land Manager, and quarterly meetings with Leadership Council and Project Funder.
Position Requirements
➢ High School Diploma or GED certificate.
➢ Community health experience or service.
➢ Excellent time management, written and oral communication, and intentional interpersonal skills that help to facilitate a culture of care and responsibility
➢ Ability to work independently and with a small team, demonstrating dependability, flexibility, and creativity in a fast-paced work environment ➢ Awareness of and respect for cultural, racial, and ethnic backgrounds and experiences within self and others
➢ Knowledge of MS Word and PowerPoint or other oral and written communication software.
➢ Knowledge of Electronic Medical Systems and Medi-Cal billing process, or open to being trained
➢ Background in working in new programs, projects, or initiatives
Preferred, but optional
➢ BA degree in psychology, public health, education, social work, or ethnic/gender studies or the equivalent of 3 years of prior relevant experience
➢ Community Health Worker Certification
➢ Understanding and knowledge of Maternal health, behavioral health, or family health
Community organizing or political education
Culturally rooted practices (e.g., doula, midwifery, food sovereignty)
Program evaluation and data use Certificates/Licenses
➢ A valid California driver’s license with proof of vehicle insurance and reliable transportation or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
