Job Description
Essential Functions
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
- Assures the appropriateness of drug therapy and compliance with hospital policies
- Oversees the distribution of medications throughout TJUH
- Supports implementation of processes to assure the quality of Pharmacy services
- Trains pharmacy technicians, residents, students, interns and new pharmacists as assigned.
- Precepts student pharmacists AND residents as assigned. See Preceptor Development & Evaluation
Competencies (Knowledge, Skills, And Abilities Required)
- Knowledge of and ability to apply pharmacokinetic principles for monitoring and dosing
- Monitor patient response to medications and provide recommendations on management if appropriate
- Identify and document sentinel/serious events, near misses, medication errors and adverse
- Provide quality patient care in accordance with standards of practice, demonstrating high
- Promote and assure compliance with the hospital formulary and policies and procedures
- Provide evidence-based recommendations to support patient care
- Doctor of Pharmacy
- Completion of PGY2 Pharmacy Specialty Residency; OR
- Completion of PGY1 Pharmacy Residency PLUS 2 years of relevant experience; OR Completion of no residency PLUS 5 years of relevant experience
- Pharmacist License in the State of Pennsylvania
