- Recertification: $110
- Discharge OASIS: $95–$10
- Evaluation: $95–$100

What You’ll Do
- Conduct comprehensive in-home occupational therapy evaluations
- Develop individualized treatment plans tailored to functional needs
- Implement therapeutic interventions to improve daily living skills (bathing, dressing, cooking, mobility, etc.)
- Educate patients and caregivers on adaptive strategies, safety techniques, and equipment use
- Monitor progress and update plans as needed
- Document all care accurately and in a timely manner
- Collaborate closely with the interdisciplinary clinical team to ensure cohesive, high-quality care
Qualifications
- Graduate of an accredited Occupational Therapy program
- Active Arizona OT license (or eligibility to obtain)
- Minimum 1 year of OT experience
- Current CPR certification
- Valid driver’s license, reliable transportation & auto insurance
- Strong communication and documentation skills
