Student and Family Engagement Case Manager

12 month non-exempt position, 8:00-3:00 Monday-Friday

Job Purpose: The position is responsible for providing support to help students stay enrolled in school through graduation and works closely with the Student Success Coordinator, Attendance Manager, Student Liaison, and campus based counselor/social worker to address mental and behavioral health issues that impact student achievement.

Position Qualifications: (minimum)

· Bachelor’s degree required, Master’s in Social Work (LMSW) with licensure in the State o f Louisiana preferred;

· A minimum of three years of post-degree experience in case management, providing social work services, in particular working with children, youth and families;

· Effectively communicate the needs of students and families (verbal and written).

· driving record with no moving violations, accidents, driving -related convictions or points from the past 5 years

Essential Job Duties & Responsibilities

· Monitors school attendance and develop methods to promote school attendance;

· Provide home visits to meet families to address barriers to learning;

· Interviewing students individually, in families, or in groups, assessing their situations, capabilities, and problems, to determine what services are required to meet their needs;

· Provide direct support, coaching, mentoring and intervention to increase youth self -sufficiency and goal attainment

· Provide crisis intervention, counseling services and the coordination of provided services from various support agencies;

· Gathers background information on the student’s social history by conducting behavioral observations,

· Help families understand school policies, services and programs;

· Assist teachers and principals in developing plans to help students with academic and behavior barriers;

· Follow up on student issues referred by parent, teacher, administrator or students;

· Member of the Positive Behavior Intervention and Support team/RTI behavior support team;

· Maintain confidentiality of student and family issues and records;

· Counseling students, groups, families, or communities regarding issues including mental health, poverty, unemployment, substance abuse, physical abuse, rehabilitation, social adjustment, child care, or medical needs;

· Consulting with families, teachers, and other school personnel to determine causes of problems such as truancy and misbehavior, and to implement solutions;

· Serving as a liaison between students, homes, schools, family services, child guidance clinics, courts, protective services, doctors, and other contacts, to help children who face problems such as disabilities, abuse, or poverty;

· Supports the school’s mental wellness program, including providing targeted support at tiers 2 and 3 for social and emotional learning (SEL) intervention and consultation for tier I SEL instruction;

· Facilitate evidence-informed training such as Darkness to Light, Parenting Classes, Cultural Sensitivity and others deemed necessary to better protect students and families;

· Conduct small groups as it relates to social and emotional learning;

· Serves as a resource to teachers, staff and parents regarding family engagement, trauma support and the academic/social needs of students, including recommendation of interventions;

· Serves as a liaison between the student, home, school, private counseling facilities and community resources such as social services, court services, McKinney-Vento, and family service agencies;

· Develops a comprehensive plan to ensure maximum parent and family engagement;

· Coordinates a minimum of six (6) parent outreach and engagement activities annually;

· Complete documentation including, but not limited to: care and services plans, needs assessments, progress notes and protocols for crisis-oriented measures;

· Research and advocate for appropriate public assistance resources for families.

Job Type: Full-time

Pay: $48,250.00 - $63,250.00 per year

Education:
