The Senior Outreach Worker identifies isolated or home-bound seniors and provides or locates services that allow them to maintain their independence.

Full time, Non-Exempt Position

Location: Remote Work from Home - Must Reside in Morrisonville, Schuyler Falls, Peru, or Keeseville

Hourly Rate of Pay: $19.05

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Locates seniors in need of service through outreach, community programs, and other sources.
  • Provides information about services, benefits, and entitlements available for seniors.
  • Links clients with service providers and community resources.
  • Provides case assistance, including advocacy and collaboration with other service agencies.
  • Remains knowledgeable about changes in services effecting seniors.
  • Submits monthly reports, including input of services into Captain and OFA Data programs on a timely basis.
  • Travel required within service areas.
  • Attends all required staff meetings and training.
  • Complies with all health and safety regulations.
  • Performs all duties in a manner consistent with the mission and goals of the Joint Council for Economic Opportunity, Inc.
  • Performs all other related duties as assigned.

EDUCATION And/or EXPERIENCE

High school diploma or general education degree (GED) and, six months of related experience and/or training; or, an equivalent combination of education and experience.

CERTIFICATES, LICENSES, REGISTRATIONS

The employee in this position must possess and maintain a valid New York State driver’s license and an acceptable driving record. As a condition of employment, employees in positions requiring driving will be enrolled in the New York State Department of Motor Vehicles License Event Notification Service (LENS) program.
