Position Summary:
We are seeking a dedicated and experienced Case Manager to lead and coordinate the intake, admissions, discharge planning, and overall wellness of individuals served. This role also includes proactive outreach and referral development to grow our member base and ensure access to care for those in need. The ideal candidate will be passionate about holistic support and skilled in building relationships across healthcare and community networks.
Key Responsibilities: Outreach & Referral Development:
- Identify and engage potential referral sources including hospitals, halfway houses, shelters, clinics, and community organizations.
- Build and maintain strong relationships with referral partners to promote program awareness and member recruitment.
- Represent the organization at community events, networking opportunities, and outreach initiatives.
Intake & Admissions:
- Conduct initial assessments and interviews with prospective members.
- Manage the admissions process, ensuring timely and accurate documentation.
- Collaborate with interdisciplinary teams to determine eligibility and develop service plans.
Case Management:
- Develop, implement, and monitor individualized care plans tailored to each member’s needs.
- Maintain regular contact with members to assess progress and adjust plans as needed.
- Advocate for member needs and connect them with appropriate resources and services.
Discharge Planning:
- Prepare and execute discharge plans in collaboration with members, families, and service providers.
- Ensure continuity of care and appropriate follow-up services post-discharge.
- Document discharge summaries and outcomes.
Wellness Oversight:
- Monitor the physical, emotional, and social wellness of all members served.
- Identify and address wellness concerns proactively.
- Coordinate wellness programs and initiatives to promote holistic health and recovery.
Qualifications:
- Minimum of 3 years of experience in case management, social work, or a related field.
- Bachelor’s degree in social work, Psychology, Human Services, or related discipline (Master’s preferred).
- Proven experience in outreach, referral development, or community engagement.
- Strong understanding of intake/admissions and discharge processes.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work collaboratively in a multidisciplinary team environment.
- Familiarity with electronic health records and case management software (Therap).
