Title: Registered Nurse - Hospital-Based Liaison (Hospice)

Location: Covering Bergen, Passaic, and Hudson counties

We are seeking an experienced Registered Nurse to serve as a hospital-based liaison, coordinating high-quality hospice care for patients across various healthcare facilities in Bergen, Passaic, and Hudson counties. This full-time role offers a manageable caseload of 4–6 patients per day and runs Monday through Friday, 8:30 a.m. to 5:00 p.m., with on-call duty one weekend per month and two major holidays per year (one winter, one summer).
Responsibilities:
- Assess, plan, and implement nursing care in alignment with care plans and physician orders.
- Document all care activities accurately and timely within EMR systems.
- Identify payer sources, maintain authorizations, and coordinate interdisciplinary care.
- Supervise and evaluate home health aide staff, ensuring standards of care are met.
- Collaborate with clinical managers, physicians, and facility teams to optimize patient outcomes.
- Provide patient and family education using tailored teaching strategies.
- Maintain productivity requirements while ensuring the quality and safety of care.
- Participate in staff education and stay current with best practices.
Qualifications:
Required:
- Associate Degree in Nursing or Diploma Certificate in Nursing
- At least 6 months of RN experience or 1 year of LPN experience (home care or hospice preferred)
- Valid NJ RN License
- BLS Certification (AHA)
- Proficient in PC use and documentation
Preferred:
- Bachelor of Science in Nursing (BSN)
- IV Certification
