Description
Summary: Provides group/individual counseling, therapy and psychiatric rehabilitation recovery services to consumers with psychiatric disabilities who are enrolled in the Personalized Recovery-Oriented Services (PROS) program. Educates and develops participant readiness for recovery and facilitates the establishment of self-directed change/outcomes (e.g. obtaining desired employment/moving to preferable housing, etc.) by performing the following duties.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Conducts individual and group counseling/teaching by facilitating specific Personalized Recovery Oriented Services curriculum.
- Provides on-going counseling/therapy and support related to mental health impairments, specific to needs of each individual participant. Coaches and uses motivational techniques to help consumers develop to their potential.
- Utilizes psychiatric rehabilitation techniques to deliver services in the area of readiness determination, goal setting, functional and resource assessment, service planning, skill and resource development and discharge planning.
- Collects, organizes, and analyzes information about participants through records, tests, interviews, and professional sources, to appraise their interests, aptitudes, abilities, and personality characteristics for recovery planning.
- Completes all mandated reporting requirements by Jawonio and regulating agencies including but not limited to the following:
- quarterly service plans
- individual and group progress notes
- progress, incident, discharge, etc. reports
- Medicaid/Medicare required reports and documentation
- Agency related documentation
- Helps ensure appropriate safeguards for participants are developed and maintained.
- Reports unique or emergency medical conditions of participants to the program nurses and PROS management team.
- Embraces team approach, attending and participating in all staff meetings and mentoring new staff.
- Models appropriate/acceptable social and work skills.
- Provides crisis intervention as assigned/needed when a participant is experiencing difficulties.
- May accompany participants on trips outside the agency or to drive participants to and from meetings.
- Communicates and works with participants, their families and professional staff (both internal and external) to help ensure participant growth and recovery.
- Attends all required interdisciplinary and team conferences and special meetings both in and outside of the agency to determine appropriate programs and recovery plans for participants.
- Provides individual and group coverage for program participants as needed when other PROS staff are unavailable.
- Maintains safe and orderly work environment and work area.
Qualifications
Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education/Experience: Master’s degree in Social Work is required. Must meet NYS Office of Mental Health guidelines for professional staff as qualified by possessing any of the following credentials, with associated training and experience: NYS-certified Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
Additional Requirements:
- Daily access to an automobile is required. A valid NY State Driver’s License and ability to drive and driving record acceptable for agency insurance coverage are required.
- Remain current in all OMH and agency required trainings
- Candidates must be cleared and maintain acceptable record under the NYS Justice Center and NYS OMH mandated criminal background check process.
- Must be clear and maintain an acceptable record under the Medicaid Fraud & Abuse clearance process.
- Regular work schedule: Monday to Friday – 9:00 to 5:00pm. Requires flexing regular schedule to accommodate meeting and event times including occasional evening or weekend work.
Salary : $57,385 - $73,055
