Field Patient Care Coordinator (Commission-Based)
We are seeking a Field Patient Care Coordinator to work directly with patients in a community-based setting. This role focuses on patient engagement, education, and support related to weight management and wellness programs.
Responsibilities
- Visit assigned patient locations in the field
- Communicate with patients in a professional, supportive manner
- Provide basic education and guidance on weight loss and wellness programs
- Explain program options and help patients understand next steps
- Document patient interactions and report updates to the care team
- Maintain professionalism and respect for patient needs and mobility limitations, including patients who may require wheelchair assistance or additional support
Compensation
- This is a commission-based role tied to pre-paid patient enrollments or completed program cases
- Pay is structured as compensation for patient education, enrollment support, and completed engagements

Requirements
- Comfortable working in a field-based, patient-facing role
- Strong communication skills and ability to build trust with patients
- Reliable transportation and ability to travel locally
- Ability to work independently
- No medical license required, but healthcare, wellness, or customer-facing experience is helpful
