Retreat Resident Care Coordinator

The Retreat Nursing Home is hiring for a Resident Care Coordinator position.

Position Summary:

Responsible for coordinating resident care services, ensuring regulatory compliance, facilitating communication among interdisciplinary team members, and supporting resident-centered care planning.

Collaborates with annual updates to the facility assessment

Assist with orientation and education of nursing personnel as assigned.

Assist nursing staff with any new admissions, hospital returns and discharges to ensure accuracy of all orders and all discharge needs are met. Enters all orders on new admissions and hospital returns

Ensure that all resident activities of daily needs are met. This includes but is not limited to resident bathing, nail care, shaving, toileting. Perform audits and education with CNA’s to ensure that ADL needs are met.

Assist with restorative nursing program as necessary or requested.

Identify documentation deficiencies or inconsistencies and works with nursing staff to obtain appropriate follow-up or correction.

Perform nursing cart audits to ensure compliance as requested.

Assist with physician and family notifications for laboratory and x-ray orders

Provides charge nurse and/or treatment nurse support for staff call outs or as requested.

Provides weekend supervisor support as necessary.

Participate in the Nursing Administrative on call rotation.

Assist with CNA group/shift daily assignments as needed

Assist with coordination of physician/provider visits, including scheduling transportation.

Back up Scheduler when Scheduler is out. Communicate with DON/ADON regarding any missing shifts and coordinate with nursing staff and contracted agencies to fill those positions.

Assist nursing department with ordering and distributing supplies as needed or requested.

Assist with ordering Medicare Part B supplies, supplements, OTC medications and any other supplies as needed.

Knowledge, Skills, and Abilities

The Resident Care Coordinator is expected to:

Treat all residents with dignity and respect. Promotes and protects all residents’ rights.

Establishes a culture of compliance by adhering to all facility policies and procedures. Complies with standards of business conduct, and state/federal regulations and guidelines.

Follows appropriate safety and hygiene measures at all times to protect residents and themselves.

Maintains confidentiality of protected health information, including verbal, written, and electronic communications.

Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel. Reports any retaliation or discrimination to HR or compliance officer.

Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and/or administrator. Protects residents from abuse and cooperates with all investigations.

Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy.

Participates in all life safety and emergency drills and trainings. Fulfills responsibilities as assigned during implementation or activation of the facility’s emergency plan.

Reports work-related injuries and illnesses immediately to supervisor.

Follows established infection control policies and procedures.

As a condition of employment, completes all assigned training and skills competency.

Remains current on new developments by attending professional institutions, reading professional journals, attending professional seminars, or reading new laws, rules, and regulations.

Uses proper lifting and body mechanics while delivering care to residents.

Performs administrative and/or other duties as assigned.

Serves on, participates in, and attends facility, departmental, or committee meetings as required or appointed.

Minimum requirements include the following:

A nursing degree from an accredited college or University or be a graduate of an approved LPN program.

Two years of experience as an LPN.

Current, unrestricted nursing license in Georgia

Current CPR/BLS certification

Knowledge of current standards of practice and the rules, regulations, and laws related to the long-term care industry. Ability to teach this knowledge to others.
