ACCOUNTABLE FOR:
The Retreat Licensed Practical Nurse is responsible for age appropriate assessment, planning, implementation, and evaluation of residents. Responsible to assure the planned care is delivered in a safe, economical and efficient manner, which follows facility’s policies and procedures
GENERAL PURPOSE AND ESSENTIAL FUNCTIONS:
Responsibilities and Duties:
- Receive report and count with off-going nurse. Be prepared at the end of the shift to give report to the on-coming nurse unless you are in an emergency.
- Comply with State and Federal Rules and Regulations for long term care facilities; as well as follow facility policies and procedures.
- Supervise, teach, and assist C.N.A.s as necessary. Note that all residents are being turned and positioned, cleaned and dried, etc.
- Acquire general knowledge of pertinent facility and nursing policies and other department’s functions and interactions with nursing service.
- Function as an important member of the nursing team, receiving assignment from and reporting to supervisor(s) as indicated.
- Does work accurately, neatly, and completely.
- Maintains organization at the nurse’s station.
- Communicates resident concerns, problems, and / or changes in condition to the physician.
- Provides privacy for all residents.
- Complete admissions, discharges, and transfers of resident forms as needed.
- Answers resident’s call light and seeks assistance as necessary.
- Observes and reports patient’s general condition and behavior, both physical and mental.
- Administer medications as ordered with each medication pass and document in the EHR.
- If indicated, complete treatments as ordered and document in the EHR.
- Checks to assure the C.N.A.s have completed all duties and POC by the end of the shift.
- Document all pertinent information in the EHR.
- Complete incident reports, event fall forms, and observation forms in the EHR each shift and report to DON/ADON as necessary.
- Knows when and how to administer CPR and does so.
- Complete Report Sheet and Charge Nurse Checklist each shift.
- Update the schedule and daily assignment as needed.
- Assist physicians when at facility and via phone.
- Ensure 2 step PPD is done on admission and results documented on the EHR.
- Orders OTC medications as needed and keeps med room organized.
- Assist the DON, ADON, and Administrator with duties and projects as assigned and maintain confidentiality of all records and issues in the facility.
- Receive visitors, answer telephone, and answer call lights in a courteous manner.
- Fax pharmacy reorder sheets as needed.
- Assure that all residents receive meals and are assisted with their meals as needed.
- Receive, verify, and put away all medications delivered by pharmacies.
- Responsible for communicating with the doctors and family members regarding changes in residents.
- Responsible for recording I & O’s in the EHR as ordered.
- Responsible for carrying out all physician orders in a timely manner and notifying physician if there is a reason the order cannot be carried out.
- Responsible for documenting glucose levels in the EHR and in the accu check book and notifying physicians as ordered.
- Stock medication carts at the end of your shift.
Related Functions:
- Arrives for the scheduled shift no earlier than fifteen (15) minutes before the shift begins and clocks out no later than fifteen (15) minutes after the scheduled shift ends.
- Required to work weekends on a rotating basis.
- Responsible for one overtime shift a month if needed.
- Demonstrates loyalty to the organization, honesty, and integrity at all times in the care and use of patient / resident and facility property.
- Demonstrates positive support of any and all programs / projects implemented by the DON or Administration.
- Maintains confidentiality of patient / resident information.
- Displays a neat, clean, and professional appearance at all times.
- Cooperates when needed if the facility faces an emergency or unusual circumstance.
- Participates in Continuous Quality Improvement activities through monitoring and evaluation of resident care.
- May be asked to participate on committees and community projects.
- Participate in staff meetings, in-service programs, performance improvement, and infection control activities as necessary.
- Promote teamwork with physicians, residents, families, visitors, and co-workers.
- Use proper body mechanics for transfers, lifting, etc…
- All other duties as assigned.
