Jasmine Estates Memory Care is seeking a dedicated and highly professional Resident Care Coordinator (RCC) to join our leadership team. The ideal candidate will be a compassionate, detail-oriented individual with the ability to thrive in a fast-paced environment and support exceptional care for our residents.
Minimum Requirements
ACMA certification with Glucose & Insulin certification (required)
Strong organizational, time-management, and multitasking skills
Ability to work under pressure and adapt to changing priorities
Excellent interpersonal and communication skills
Professional and team-centered work ethic
Commitment to going above and beyond the typical workday when needed
Key Responsibilities
Manage and maintain the clinical staff schedule, ensuring adequate coverage and smooth operations
Coordinate closely with Hospice partners, the Wellness Director, and other providers to promote excellent care outcomes and quality
Oversee and coordinate care for four resident halls, ensuring consistency and adherence to care standards
Update clinical communication logs, resident communication sheets, and related documentation in a timely and accurate manner
Order medications, track deliveries, and conduct routine med cart audits for compliance and accuracy
Train, coach, and support clinical staff in accordance with company guidelines, expectations, and best practices
Assist on the floor as needed, including providing hands-on support to ensure resident needs are met
Collaborate with families, staff, third-party providers, and the Medical Director with professionalism and compassion
