A brief overview of Responsibilities:
- Conduct comprehensive psychosocial assessments within 5 days of admission
- Assess and address emotional, social, financial, and environmental needs, identifying appropriate interventions
- Participate in Interdisciplinary Group (IDG) meetings to develop and revise individualized plans of care
- Obtain physician orders in accordance with patient needs and agency protocols
- Monitor, document, and communicate changes in patient and family psychosocial status
- Maintain complete and accurate clinical records, including assessments, care plans, and progress notes
- Evaluate the effectiveness of social work interventions and adjust care plans as needed
- Coordinate weekly visit schedules in collaboration with Case Managers
- Participate in Quality Assessment and Performance Improvement (QAPI) initiatives
- Provide ongoing support to the IDG, even if social work services are initially declined by the patient or family
- Assist patients and families with coping strategies and connecting to community resources
- Perform additional responsibilities as assigned by agency leadership
Qualifications:
- Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education
- At least one year of Hospice experience is preferred.
- The ability to communicate well, both verbally and in writing.
