James River Home Health is hiring! We're looking for a compassionate and dedicated Home Health LPN to become part of our expanding team serving the Tri-Cities area. This role is ideal for nurses who enjoy using their clinical skills independently while building meaningful relationships with patients. You'll provide skilled nursing care in the home, including wound care, catheter care, ostomy care, disease management, patient education, and ongoing patient assessment.
Home Health experience is preferred but not required. We welcome LPNs with strong clinical experience from skilled nursing facilities, rehab hospitals, long-term care, subacute rehab, acute care hospital settings, LTAC, assisted living treatment teams, wound care clinics, dialysis, and other skilled nursing environments.

Brief Overview of Responsibilities:
As a Home Health LPN, you'll provide skilled nursing care to patients in their homes including wound care, catheter care, ostomy care, disease management, patient education, and ongoing monitoring. You'll work closely with the RN, physician, patients, and families to ensure high-quality care while helping patients maintain their independence and remain safely at home.
Qualifications:
• Current Virginia Licensed Practical Nurse License
• Graduate of an Accredited School of Nursing
• Home Health Experience Preferred but Not Required
• Experience with wound care, trachs, feeding tubes, ostomies, catheter care, disease management, and other skilled nursing services is highly preferred
• Valid Driver's License and Reliable Transportation Available During Work Hours
