A brief overview of Responsibilities:
- Complete a spiritual assessment within 5 days of admission.
- Support and help develop the patient’s interdisciplinary care plan related to spiritual needs.
- Provide ongoing spiritual care that is based on the patient’s and family’s wishes, preferences, and beliefs.
- Collaborate with local clergy when requested by the patient.
- Maintain required documentation, including evaluations, care plans, and visit notes.
- Monitor and evaluate the effectiveness of spiritual counseling for the patient and family.
- Coordinate weekly visit schedules with Case Managers to align care.
- Participate in IDG and QAPI meetings as needed.
- Lead or support prayer, worship, and spiritual rituals when appropriate.
- Perform additional duties as assigned.
Qualifications:
- Has a degree in theology or from a seminary.
- Comfortable in an interfaith setting.
- Able to accept different lifestyles, cultures, beliefs, and values.
- Must have a valid driver's license and a reliable car available for use during work hours.
