At Jacksonville Health and Rehabilitation, we're looking for compassionate and dependable Licensed Practical Nurses (LPNs) to join our Day Shift nursing team.

We understand that one schedule doesn't fit every nurse. That's why we're offering multiple scheduling options, including 8-hour, 12-hour, 16-hour, and 7-on/7-off schedules, based on availability and facility needs.

What You'll Do

As an LPN Staff Nurse, you'll provide direct nursing care while working closely with RNs, CNAs, physicians, and the interdisciplinary care team to ensure residents receive safe, compassionate, and individualized care.

Key Responsibilities:

• Provide nursing care in accordance with individualized resident care plans and physician orders
• Administer medications and treatments accurately and safely
• Monitor residents for changes in condition and report concerns promptly
• Complete accurate and timely clinical documentation
• Assist with admissions, transfers, and discharges
• Communicate effectively with residents, families, physicians, and interdisciplinary team members
• Provide guidance and direction to CNAs as appropriate
• Participate in resident care planning and support positive clinical outcomes
• Follow infection prevention, safety, and regulatory compliance procedures
• Promote resident dignity, independence, comfort, and quality of life
• Maintain professional nursing standards and facility policies

What We're Looking For

• Current Licensed Practical Nurse (LPN) license in good standing
• Long-term care, skilled nursing, or rehabilitation experience preferred
• Strong nursing assessment and communication skills
• Ability to recognize and report changes in resident condition
• Excellent organizational and documentation skills
• Dependable, professional, and team-oriented approach to nursing
• Compassionate attitude toward elderly and rehabilitation residents
• Commitment to providing safe, high-quality, resident-centered care

A predictable rotation with extended stretches of scheduled time off.
