Minimum Qualifications
- Associate degree in Physical Therapy from an accredited program.
- Active Illinois PTA licensure required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Minimum education: Associate degree.
- Client requirement: experience or ability to work in home health settings.
- Candidate must be able to document visits electronically; familiarity with HCHB is preferred.
Preferred Qualifications
- Home Health clinical experience preferred.
- Experience using HCHB electronic documentation preferred.
Assignment Details
- M-F schedule, 8:00 AM – 4:30 PM.
- Coverage across the agency service area; travel between patient homes required.
- Mileage reimbursed between visits from first patient to last patient.
