Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Current Virginia Physical Therapist license required to start.
• Physical Therapist checklist required to start.
• Hepatitis B, TB, and Tdap required to start.
• Criminal Background, OIG Search, Statement of Good Health, OSHA Packet, Verification of Professional Education, I-9 Form, GSA Search, and UDS - 9 Panel (Marijuana Not Included) required to start.
• Contract Therapist Compliance Attestation required to start.

Assignment Details

• Schedule structure: 5x8s.
• Clinic hours: Monday–Thursday, 7:00 AM–7:00 PM; Friday, 7:00 AM–5:00 PM.
• Appointments are scheduled every 30 minutes, with an average treatment time of 60 minutes.
• Expected productivity includes 12 visits per day, 90% productivity, and 55 visits per week.
• Documentation system: NextGen.
• Floating to other locations may be required.
• Dress code: Business Casual.
