You will plan, implement, and evaluate individualized physical therapy care plans in patient homes, collaborate with the interdisciplinary team, and provide education and training to patients and families. Minimum Requirements
- Bachelor of Science in Physical Therapy from an APTA-approved program, or Master’s or Doctoral degree in Physical Therapy.
- Current state license as a Physical Therapist. Must be active to start the assignment.
- Proof of current CPR certification.
- Current driver’s license, reliable transportation, and insurance.
- 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting. This experience is required.
- Ability to perform prolonged standing and walking, heavy lifting, bending, pushing, and pulling as part of patient care.
- Ability to travel locally and work a flexible schedule.
- Must read, write, and speak fluent English.
- Client requirement: Experience with durable medical equipment and ability to order/adapt rehabilitative equipment for home environments.
Preferred Qualifications
- Previous home care experience preferred.
- Demonstrated commitment to continuing education and professional growth preferred.
Assignment Details
- Work in home health setting providing in-home physical therapy services.
- Ability to travel locally and float as needed within the client’s service area.
