Minimum Requirements
- Associate, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Alabama or multistate Physical Therapist license required prior to start of assignment.
- Valid driver’s license required.
- Current BLS/CPR certification from AHA or ARC required.
- Home Health experience preferred.
- Ability to perform patient care and physical therapy duties in home health settings.
- Professional Physical Therapy certification as required by role.
Assignment Details
- Facility Type: Home Health.
- Shift Name: Evenings.
- Shift Details: Evenings 2000-0400.
- Coverage Areas: Eufaula and Dothan, AL.
- Weekly Hours Guarantee: 40 hours.
- Productivity Expectation: 30 points.
- Weekend Requirement: Occasional weekend work.
- Mileage reimbursed between patient visits from first patient to last patient.
- Dress Code: Navy scrubs and closed-toe shoes.
