Minimum Requirements
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License is Required to Start the Assignment
- BLS Certification May Be Required from AHA or ARC
- OASIS experience required
- Home health experience required
- Hospice experience required
- Experience traveling from patient to patient for home visits required
- Ability to manage and adjust to schedule changes required
Assignment Details
- Working hours: 8:00 AM - 5:00 PM
- Shift days: Monday through Friday
- Holiday coverage as needed
- EMR system: HealthWyse
- Outpatient care
