Job Description
This position exists to work independently and as part of a team responsible for reconciling patients' medication regimens during the admission process. Medication reconciliation is defined as obtaining a complete and accurate medication history with the involvement of the patient and/or family; comparison of a patient's home medications to the medications ordered on admission and identification of discrepancies, followed by a clarification of any noted discrepancies.
Requirements
Education:
High school diploma or equivalent
College courses desirable, but not required
Experience:
Prefer two years in clinical setting. Pharmacy experience preferred
Licenses, Certifications and/or Registrations:
harmacy Technician registration through Alabama Board of Pharmacy
National Technician Certification recognized by ALBOP
Equipment/Tools/Work Aids:
Computer, Calculators, Drug information software, EMAR, Telephone, and copier
Physical Requirements:
Manual dexterity and visual acuity necessary to utilize the PC and calculator
Visual acuity also necessary for filing and proofing
Ability to sit or stand for prolonged periods
Verbal communication and hearing ability to communicate with a multitude of customers; ability to meet the attendance requirements of the job
