Case Manager - Ryan White Programming
Summary: Under the general supervision of the HIV Division Director, perform case management, case findings, and outreach duties of a professional nature within a local health department for Person's Living With HIV/AIDS (PLWHA). Promotes and provides a supportive environment to meet the needs of a diverse client base.
Hours: 8:00 am – 4:00 pm Monday – Friday: Full-time 35 hours per week
Some evenings/weekends, as needed
Salary: $21.00/hour – $30.00/hour
Job Specifications:
Education:
Bachelor's degree in Social Work (BSW), related human service, social science, or health education field from an accredited college or university, with licensure in mental health counseling and Master's degree in Social Work (MSW);
OR
Associate's degree in a related human service, social science, or health education field from an accredited college or university, plus 4 years of case management or related experience. OR
Demonstrated knowledge and at least 6 years of work experience equivalent to the academic credentials outlined above;
OR
Current Illinois licensure as a licensed practical nurse (LPN), registered nurse (RN), or advanced practice registered nurse, with Case Manager and Association of Nurses in AIDS Care (ANAC) certification preferred.
Valid driver's license, proof of insurance, and reliable transportation.
Duties and Responsibilities:
Assess health, social and educational needs and formulate plans to meet those needs.
Provide advocacy, referrals and on-going follow-up to appropriate providers within the community for services needed.
Participate, evaluate and implement QM project client level data and report outcomes.
Provide home visits to clients as needed and provide outreach services in the community as required.
Provide HIV transmission education and risk reduction information to the clients.
Assist clients in developing skills to become more independent and to access social service agencies and health needs.
Complete paperwork and client files (paper and electronic) as required by the State and the local public health department on a timely basis and with high degree of accuracy.
Assist in the planning and delivery of community events and outreach activities designed to improve the knowledge of general population, health care providers and social service agencies.
Incorporate ethical standards of practice into all interactions, including the use of data and information.
Skills:
Requires proficiency in basic computer skills. Prefer knowledge and skills in database management.
Requires some travel both regional and state-wide.
Excellent communication skills, oral and written.
Extremely organized, with good time management skills to meet deadlines.
Passion for promoting public health and improving the well-being of the community.
Demonstrated ability to present and conduct self professionally; a commitment to confidentiality; and a willingness to work as part of a team.
Flexible, strong interpersonal skills and willingness to adapt to changes in the work environment.
