Qualified Professional (QP) – 1915(i) Services Job Description (North Carolina)
Position Title: Qualified Professional (QP) – NC Medicaid 1915(i) Services
Reports To: Clinical Director / Program Manager
Position Summary:
The Qualified Professional (QP) is responsible for the oversight, coordination, implementation, and monitoring of NC Medicaid 1915(i) Home and Community-Based Services for individuals with mental health disorders, substance use disorders, intellectual/developmental disabilities (I/DD), and traumatic brain injuries (TBI). The QP ensures services are delivered according to person-centered plans, Medicaid requirements, and NC Department of Health and Human Services (NCDHHS) standards. 1915(i) services are designed to help individuals remain in their homes and communities while developing independent living, social, vocational, and community integration skills.
Essential Duties and Responsibilities
- Conduct assessments and participate in person-centered planning meetings.
- Develop, implement, and monitor individualized service plans based on identified needs and goals.
- Provide clinical and administrative oversight to paraprofessionals, direct support staff, and other service providers.
- Ensure service delivery complies with NC Medicaid 1915(i) policies, regulations, and documentation standards.
- Review and approve service documentation, progress notes, and treatment updates.
- Coordinate care with families, guardians, healthcare providers, schools, community agencies, and other stakeholders.
- Monitor client progress toward established goals and modify interventions as needed.
- Ensure individuals receive services in the least restrictive environment and promote community integration.
- Maintain accurate and timely records in accordance with Medicaid, state, and agency requirements.
- Participate in utilization reviews, audits, quality assurance activities, and staff training.
- Provide crisis intervention and support when necessary.
- Advocate for individuals' rights, independence, and self-determination.
Required Qualifications
To qualify as a Qualified Professional (QP) in North Carolina, an individual must meet one of the following criteria:
- Hold a license, provisional license, registration, certification, or permit in a human services field recognized by North Carolina; or
- Possess a Master's degree in a human services field and at least one year of supervised experience with the population served; or
- Possess a Bachelor's degree in a human services field and at least two years of supervised experience with the population served; or
- Possess a Bachelor's degree in a non-human services field and at least four years of supervised experience with the population served.
Preferred Qualifications
- Experience working with Medicaid-funded behavioral health or I/DD services.
- Knowledge of NC Medicaid 1915(i) services and Tailored Plan requirements.
- Experience with person-centered planning and community-based service delivery.
- Strong documentation, communication, and organizational skills.
- Valid North Carolina driver's license and reliable transportation.
Knowledge, Skills, and Abilities
- Knowledge of NC Medicaid policies, service definitions, and documentation requirements.
- Ability to supervise and mentor direct care staff.
- Strong crisis intervention and problem-solving skills.
- Ability to maintain professional boundaries and confidentiality.
- Proficiency in electronic health record (EHR) systems and Microsoft Office applications.
- Ability to work independently and as part of an interdisciplinary team.
Working Conditions

- Frequent travel within assigned service areas.
