What are the responsibilities and job description for the Paraprofessional position at J4 Stability & Safe Haven?
The Paraprofessional provides direct support services to individuals with mental health needs in home and community-based settings. Services include Individual Support, Respite Care, and Community Living Supports I (CLS I) in accordance with North Carolina Medicaid service definitions, the individual’s Person-Centered Plan (PCP), and all applicable state and agency policies. The Paraprofessional works under the supervision of a Qualified Professional (QP).
Essential Duties and Responsibilities
- Provide Individual Support to assist individuals in achieving goals outlined in the Person-Centered Plan (PCP)
- Deliver Respite Care to support caregivers by ensuring the individual’s health, safety, and supervision
- Implement Community Living Supports I (CLS I) to promote skill development, independence, and community integration
- Assist individuals with daily living activities, including but not limited to:
- Personal care and hygiene support (as authorized)
- Meal preparation and nutrition support
- Household tasks and organization
- Medication reminders (no administration unless certified/authorized)

- Support individuals in community participation, including appointments, social activities, and skill-building outings
- Provide behavioral support using trauma-informed and recovery-oriented practices
- Utilize de-escalation techniques and follow crisis intervention protocols when needed
- Maintain accurate, timely documentation, including service notes, incident reports, and progress toward goals
- Observe and report changes in behavior, mental status, or safety concerns to the QP or supervisor
- Ensure compliance with HIPAA, confidentiality requirements, and client rights
- Follow all NC Medicaid, LME/MCO, and agency policies and procedures
- Participate in required trainings, supervision, and team meetings
Minimum Qualifications
- High School Diploma or GED required
- At least 18 years of age
- Ability to pass:
- Criminal background check
- DMV check (if transporting individuals)
- Drug screening

- Valid NC driver’s license and reliable transportation (if applicable)
- Ability to complete required NC Medicaid and agency training
Preferred Qualifications
- Experience working with individuals with mental health needs
- Previous experience in:
- Direct support
- Residential or community-based mental health services

- CPI, NCI, or crisis intervention training
- CPR/First Aid certification (or ability to obtain)
Knowledge, Skills, and Abilities
- Understanding of mental health diagnoses and recovery principles
- Ability to implement structured interventions and follow treatment plans

- Ability to maintain professional boundaries
- Cultural competence and sensitivity

Supervision
This position is supervised by a Qualified Professional (QP) as defined by North Carolina Medicaid policy. Services are delivered in accordance with the individual’s approved Person-Centered Plan.

Pay: $15.00 - $23.00 per hour
