Provide compassionate, dependable in-home support to older adults, individuals with disabilities, post-acute care clients, and palliative or hospice clients. Working in a single assigned home, the Certified Home Health Aide will coordinate care with a registered nurse and care manager while promoting client comfort, safety, and independence.
Responsibilities
- Assist clients with personal care and activities of daily living.
- Prepare meals according to client needs and preferences.
- Provide medication reminders as directed.
- Perform light housekeeping to maintain a safe, comfortable environment.
- Offer companionship and emotional support.
- Monitor client safety and report concerns to the registered nurse or care manager.
- Document care provided accurately and consistently.
- Communicate with family members regarding client care and relevant updates.
Preferred Qualifications
- At least one year of experience in home health care.
- Completion of a state-approved home health aide training program.
- Current Certified Home Health Aide (CHHA) certification.
- Knowledge of activities of daily living support and vital signs monitoring.
- Understanding of patient safety and infection control practices.
- Experience with care documentation and mobility and transfer techniques.
- Empathy and a compassionate approach to client care.
