Registered Nurse Case Manager (Field)

Shift/Schedule:

Monday to Friday| 8 AM to 5 PM

Summary

The Registered Nurse plans, organizes, and directs hospice care and provides skilled nursing and case management services to terminally ill patients and their families. This role emphasizes community-based care, hospice philosophy, and interdisciplinary collaboration to promote comfort, dignity, and quality of life at the end of life. Care is delivered in accordance with Medicare Conditions of Participation (42 CFR Part 418), state regulations, and agency policies and procedures.

Qualifications

1. Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.

2. Current licensure in State and CPR certification.

3. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

4. Bachelor’s degree, with one (1) year of hospice care experience preferred.

5. Management experience not required. Responsible for supervising hospice aides.

6. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

7. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.

Essential Job Functions and Responsibilities

Patient Care & Clinical Assessment

  • Completes comprehensive initial, ongoing, and as-needed assessments of patients and families to determine hospice needs, including physical, psychosocial, emotional, spiritual, and environmental factors.
  • Provides skilled nursing care utilizing the nursing process and in accordance with the State Nurse Practice Act and hospice standards of practice.
  • Assesses and manages pain and symptoms using evidence-based hospice and palliative care practices.
  • Uses assessment data to determine nursing diagnoses and develop individualized plans of care that reflect patient goals and hospice philosophy.
  • Writes, initiates, and regularly re-evaluates the plan of care; revises as patient condition, needs, or goals change.
  • Implements physician orders and administers medications and treatments as prescribed.
  • Provides wound care, medication management, symptom control, and end-of-life care.
  • Initiates appropriate preventive, palliative, and rehabilitative nursing interventions.
  • Assists patients with activities of daily living and supports comfort, dignity, and self-sufficiency as appropriate.
  • Recognizes changes in patient condition and initiates timely and appropriate clinical interventions.

Case Management & Care Coordination

  • Acts as Case Manager when assigned by the Clinical Manager and assumes responsibility for coordinating care for an assigned caseload.
  • Coordinates hospice care across settings, including private homes, assisted living facilities, skilled nursing facilities, and inpatient hospice units.
  • Ensures continuity of care through effective communication with physicians, patients, families, caregivers, and interdisciplinary team members.
  • Participates in interdisciplinary group (IDG) meetings and collaborates in care planning and goal setting.
  • Ensures arrangements for durable medical equipment, medications, supplies, and services are available and appropriate.

Interdisciplinary & Supportive Care Collaboration

  • Identifies psychosocial, emotional, spiritual, and bereavement needs and communicates findings to the interdisciplinary group.
  • Coordinates referrals to social work, chaplaincy, bereavement, and volunteer services based on patient and family preferences and needs.
  • Works collaboratively with the hospice interdisciplinary group to meet the emotional and supportive care needs of patients and families.
  • Provides culturally sensitive care and respects diverse cultural, spiritual, and family traditions.

Patient & Family Education

  • Educates patients and families on disease progression, symptom management, medications, and end-of-life care.
  • Provides anticipatory guidance, emotional support, and reinforcement of hospice philosophy and goals of care.
  • Teaches self-care techniques to patients and caregivers as appropriate.

Professional Practice & Quality Improvement

  • Supervises and delegates tasks to ancillary personnel as appropriate and within scope of practice.
  • Maintains current, unrestricted Registered Nurse licensure in the State of Hawaii and required certifications.
  • Participates in continuing education, in-service training, and professional development activities.
  • Actively participates in quality assessment, performance improvement, infection control, and patient safety initiatives.
  • Fulfills accepted case assignments and agency responsibilities as directed by the Clinical Manager.

Communication & Documentation

  • Completes, maintains, and submits accurate, timely, and compliant clinical documentation in the electronic medical record.

On-Call & Emergency Responsibilities

  • Participates in rotating on-call coverage as defined by agency policy.
  • Responds to urgent patient needs, symptom crises, and death pronouncements in accordance with hospice policy and state law.
  • Provides nursing support during the active dying phase and at the time of death.

Job Type: Full-time

Pay: $46.00 - $51.00 per hour
