Hospice Access Nurse Practitioner
Clinical Access | Hospital & Facility Rounding | Family Education | GIP Support

Schedule: Monday - Thursday; four 10 hour shifts, one Saturday/Sunday 8 hour shift
Location: Field-based throughout Oʻahu

Be the Clinical Face of Hospice Access in Our Community:

Islands Hospice is seeking an exceptional Nurse Practitioner with strong clinical expertise and a genuine ability to connect with people. This is a highly visible community-based role for an NP who can build trusted relationships with hospitals and facilities while also meeting directly with patients and families during important care decisions.

This is not a traditional sales role and it is not a hospice transition-specialist position. The Hospice Access Nurse Practitioner is an advanced-practice clinical resource who rounds in the community, educates, evaluates, coordinates, and helps appropriate patients access hospice care promptly and smoothly.

We are looking for someone who is warm, energetic, polished, clinically credible, comfortable walking into a hospital or facility, and equally comfortable sitting at the bedside with a worried family.

What You Will Do:

  • Make purposeful rounds at hospitals, emergency departments, skilled nursing facilities, care homes, and other referral sources throughout Oʻahu.
  • Build relationships with case managers, discharge planners, social workers, physicians, NPs, nurses, palliative care teams, house supervisors, administrators, and facility staff.
  • Serve as a clinical resource to the community and Islands Hospice staff regarding hospice eligibility, levels of care, symptom management, referral timing, and general inpatient hospice care.
  • Meet with patients and families to explain hospice services, answer clinical questions, assess needs, and help them understand the admission process.
  • Perform advanced clinical assessments for referred patients, including patients with uncontrolled pain or symptoms who may require GIP care.
  • Initiate and enter hospice, symptom-management, and GIP-related orders within licensure, prescriptive authority, credentialing, privileges, payer requirements, and Islands Hospice policy.
  • Collaborate closely with hospice medical leadership regarding required physician certification and medical oversight.
  • Coordinate directly with the Admission Manager, Intake, and admission nurses to ensure referrals are complete, discharges are organized, and admissions occur without avoidable delay.
  • Help resolve barriers involving physician documentation, medications, DME, transportation, facility acceptance, consents, family readiness, or scheduling.
  • Provide education or in-services to hospital, facility, community, and Islands Hospice staff as needed.
  • Assist with clinical evaluations or admissions when volume is high or advanced-practice support is needed.
  • Document referral activity, next actions, outcomes, and complete reliable handoffs.

How the Weekday Team Works:

  • You will coordinate daily priorities with the Admission Manager, Intake, and admission nurses.
  • Your hospital and facility route will be driven by active opportunities, discharge needs, referral-source relationships, and education needs.
  • You will help the admission team by ensuring referrals are clinically clear, documentation is complete, and barriers are identified early.
  • Each Thursday, you will prepare and hand off a strong weekend opportunity list to the Weekend Hospice Access Nurse Practitioner and weekend admission team.

What We Are Looking For:

  • Current, unrestricted Hawaiʻi APRN license and national Nurse Practitioner certification.
  • Current Hawaiʻi prescriptive authority; controlled-substance and DEA registrations when applicable.
  • Hospice, palliative care, acute care, hospital, emergency department, case management, or serious-illness experience strongly preferred.
  • Strong clinical judgment and confidence assessing complex patients and symptom needs.
  • An exceptional personality: approachable, engaging, positive, credible, and comfortable initiating conversations.
  • Excellent family communication and teaching skills.
  • Ability to represent Islands Hospice professionally with clinical leaders, physicians, staff, patients, families, and community partners.
  • Strong organization, responsiveness, follow-through, and comfort working independently in the field.
  • Reliable transportation and ability to travel throughout Oʻahu

Pay: $130,000.00 - $145,000.00 per year

Salary : $130,000 - $145,000
