Clinical Manager- Hospice
Position Summary:
The Clinical Manager is an office-based leadership position responsible for providing clinical oversight, coordination, and management of hospice care services. This role ensures that hospice care is delivered in accordance with regulatory requirements, professional standards, and the hospice philosophy of comfort-focused, patient- and family-centered care. The Clinical Manager supervises clinical staff, supports interdisciplinary collaboration, and promotes quality, compliance, and excellence in end-of-life care.
Position Qualifications:
- Registered Nurse (RN) with current licensure in the state of practice.
- Graduate of an accredited school of nursing; BSN preferred.
- Minimum of two (2) years of management or supervisory experience in hospice, palliative care, or a related healthcare setting.
- Demonstrated knowledge of hospice philosophy, end-of-life care, and symptom management.
- Knowledge of CMS Hospice Conditions of Participation and regulatory requirements.
- Strong leadership, communication, and organizational skills.
- Current CPR certification.
- Hospice or palliative care certification (CHPN) preferred.
Physical Requirements:
- Primarily sedentary office work with frequent sitting.
- Occasional standing, walking, and bending.
- Occasional lifting of office materials or supplies up to 25 pounds.
- Occasional travel for meetings, trainings, or supervisory visits.
- Ability to use standard office equipment, including computers and phones.
Duties of position:
- Provides day-to-day clinical leadership and support to hospice field staff from the office.
- Oversees coordination and delivery of hospice and palliative care services.
- Reviews referrals and admissions for hospice eligibility and appropriateness.
- Assigns appropriate staff based on patient needs, acuity, and staff competencies.
- Reviews and evaluates clinical documentation for accuracy, completeness, timeliness, and regulatory compliance.
- Supports, and guides clinicians in symptom management, end-of-life care, and patient/family education.
- Assists clinicians with establishing immediate and long-term goals of care and individualized plans of care.
- Facilitates and participates in Interdisciplinary Team meetings to ensure coordinated, compliant hospice care.
- Ensures compliance with CMS Hospice Conditions of Participation and organizational policies.
- Conducts routine clinical record audits and communicates findings and recommendations to leadership.
- Supports survey readiness, quality improvement initiatives, and corrective action plans.
- Collaborates with the Medical Director and Director of Clinical Services regarding clinical oversight and standards.
- Assists with recruitment, interviewing, hiring recommendations, and onboarding of clinical staff.
- Assists with orientation, training, and ongoing education of clinical personnel.
- Participates in policy and procedure development, review, and implementation.
- Serves as a clinical resource for ethical issues, goals-of-care discussions, and advance care planning.
- Participates in administrative on-call rotation.
- Conducts occasional patient visits, supervisory or joint visits as needed for clinical oversight or performance evaluation.
- Promotes excellent customer service and compassionate hospice care.
- Maintains compliance with attendance and reliability standards.
- Performs other duties as assigned by the Director of Clinical Services.
- Acts as Supervising Nurse in the absence of the Director of Clinical Services
Job Type: Full-time
Pay: $120,000.00 - $140,000.00 per year
