What are the responsibilities and job description for the Admissions Coordinator position at Island Nursing Rehab Center?
QUALIFICATIONS:
Candidate must possess 1-2yrs. prior experience in long-term care setting or related field. The admissions Coordinator represents Island Nursing and Rehab Center to the community and as such, markets the facility to others. Employees must possess knowledge of Medicare/Medicaid and understanding and familiarity with PRIs, facility Federal/NYS regulations & reimbursement systems. Candidates must possess excellent communication skills and work in a cooperative manner. Must possess ability to develop and foster relationships with prospective families and residents. Must possess ability to integrate the central admissions model as well as coordinate all Admissions and oversight of admissions department.
JOB SUMMARY:
Provides information to families and referral sources regarding admission criteria and process.
Perform secretarial and other duties to alleviate administrative details and assure completion of paperwork regarding the admission, transfer, and discharge of residents.
RESPONSIBILITIES:
1. Receives incoming referrals/calls from hospital community agencies and families. Logs all referrals into the CRM begins financial review and request clinical review.
2. Interviews with prospective residents and/or families. Provides orientation and tour of facility. Provides explanation of application process including current New York State regulations.
3. Provides resident and/or family with admission agreement contract; receives copy of signed agreement to be maintained in the facility EMR (PCC).
4. Maintains communication with hospitals and other agencies for the purpose of marketing and/or follow-up on current referral applications.
5. Coordinates the intake application process that includes verifying insurance and assisting in obtaining MCO authorizations.
6. Controls various files and statistics relating to applications, admissions, discharges and bed holds. Completes monthly statistics and report along with other reports as requested.
7. Completes and distributes facility’s daily census report as required through the EMR, PCC dashboard and daily midnight census.
8. Notifies hospitals/nursing homes/family of bed availability when Admissions is required
9. Schedule and perform facility tours of potential admissions
10. Answers telephone information requests with courtesy, accuracy and respect for confidentiality. Possess good verbal and written communication skills; documents and reports clearly and concisely. Operates as an integral member of the Admission Team as required.
11. Demonstrates knowledge, respect and sensitivity of resident’s rights as identified within the Resident Bill of Rights and facility policy and mission statement. Assures confidentiality of all resident information.
12. Attends in-service education as required.
13. Embraces the Central admissions model and identifies resident needs as its operations to the Gurwin HealthCare System.
14. Other duties as requested.
