- Schedule: Full-time, Monday–Friday, 8:00 AM–5:00 PM
- Work Environment: On-site
- Travel Requirements: To other facilities if support is needed
- Ability to perform the essential functions of the position with or without reasonable accommodation
- Ability to sit, stand, walk, bend, and move throughout the facility as needed during the workday.
- Ability to use a computer, phone, and other standard office equipment for documentation and communication.
- Ability to occasionally lift or carry light office materials.
- Ability to communicate clearly with clients, team members, and outside service providers.
- Complete client assessments and create individualized care plans.
- Coordinate services with internal teams and outside providers.
- Help clients access medical, social, educational, vocational, and community resources.
- Advocate for clients and support informed treatment decisions.
- Maintain accurate documentation while following KAR standards and company policies.
Required Qualifications
- Bachelor’s degree in behavioral science, healthcare, human services, or a related field.
- Minimum of one year of post-graduate experience in a healthcare or human services-related field.
- Strong written and verbal communication skills.
- Excellent organization, time management, and problem-solving skills.
- Ability to multitask, work as part of a team, and follow established processes.
- Experience as a Targeted Case Manager in the substance use disorder field.
- Working knowledge of case management principles, healthcare management, and reimbursement.
- Medicaid knowledge.
- Current TCM CORE/SUD certification.
