Purpose of the Position

The primary purpose of the position is to provide your assigned residents with routine daily nursing care in accordance with our established nursing care procedures, and as may be directed by your supervisors.

 

Duties and Responsibilities

Administrative Functions

1. Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.

2. Report all accidents and incidents you observe on the shift that they occur.

3. Use only authorized abbreviations established by this facility when recording information.

4. Report all changes in the resident's condition to the Staff Nurse as soon as practical.

 

Admission, Transfer, and Discharge Functions

1. Ensure that the resident's room is ready for receiving the resident (i.e., bed made, admission kit available, etc.).

2. Greet and escort residents to their room.

3. Introduce residents to his or her roommate, if any, and other residents and personnel as appropriate.

4. Make resident comfortable (i.e., help put belongings away, etc.).

5. Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged.

6. Assist residents to new rooms or to the dinning room as necessary.

 

Personnel Functions

1. Follow work assignments, and/or work schedules in completing and performing your assigned tasks.

2. Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors.

3. Notify the facility when you will be late or absent from work.

4. Report all incidents, complaints and grievances made by the resident.

5. Cooperate with the inter disciplinary team, as well as other facility personnel to assure that nursing services can be adequately maintained to meet the needs of the residents.

  1. Create and maintain an atmosphere of warmth, personal interest, pleasant and positive emphasis, as well as a calm environment throughout the unit and shift.
  2. Meet with your shift's nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services.

 

Nursing Care Functions

1. Participate in and receive the nursing report upon reporting for duty. Complete rounds.

2. Assist or prompt residents with daily dental and mouth care (i.e., brushing teeth or dentures, oral hygiene, special mouth care, etc.).

3. Assist or prompt residents with bath functions (i.e., bed bath, tub or shower bath, etc.) as directed and assigned.

4. Assist or prompt residents with dressing or undressing appropriately as necessary.

5. Assist or prompt residents with hair care functions (i.e., combing, brushing, shampooing, etc.).

6. Assist or prompt residents with nail care (i.e., clipping, trimming, and cleaning the finger or toenails.).

7. Shave or prompt male residents to shave.

8. Keep hair on female residents clean shaven or prompt (i.e., facial hair, under arms, on legs, etc., as instructed.).

9. Make beds or assist residents to make beds.

10. Assist or prompt residents to change bed linens.

11. Keep incontinent residents clean and dry.

12. Check and report bowel movements and character of stools as instructed.

13. Collect specimens as instructed (i.e., urine, sputum, stools, etc.).

14. Assist residents in preparing for medical tests (i.e., lab work, x ray, therapy, dental, etc.).

15. Assist residents in preparing for activity and social programs (i.e., church services, parties, visitors, etc.).

16. Follow established policies concerning exposure to blood or body fluids.

17. Prompt residents going to or coming from appointments, activity and social programs, etc., as necessary.

18. Answer resident calls promptly.

19. Prompt residents to exercise as instructed.

20. Provide eye and ear care (i.e., cleaning eyeglasses or hearing aides, etc.) as instructed.

21. Measure and record temperatures, pulse, and respirations (TPRs), as instructed.

22. Weigh and measure residents as instructed.

23. Assist in preparing the resident for a physical examination.

24. Check residents routinely to assure that they're personal care needs are being met.

25. Perform ADL programming in accordance with each resident's individual care plan goal.

26. Observe and report the presence of sores or other health problems.

27. Observe disoriented and depressed residents. Record and report data as instructed.

28. Turn all medications found in the resident's room or possession over to the Staff Nurse.

29. Provide residents with Reality Orientation as instructed.

 

Food Service Functions

1. Perform after meal care (i.e., prompt residents to clean their hands, face, clothing, to go to bathroom, brush teeth, clean dentures, etc.).

2. Check rooms for food articles (i.e., food in proper container, unauthorized food items, etc.).

 

 

Safety and Sanitation

1. Notify the Staff Nurse of any resident leaving or missing from the facility.

2. Keep floors dry. Report spills immediately.

3. Keep excess supplies and equipment off the floor. Store in designated areas.

4. Keep resident's personal possessions off the floor and properly stored.

5. Monitor resident smoking. Follow established smoking regulations. Report all violations.

6. Keep the nurses' call system within easy reach of the resident.

7. Report all hazardous conditions and equipment to the Staff Nurse immediately.

8. Follow established safety precautions in the performance of all duties.

9. Report all safety violations.

10. Before leaving work area for breaks, or at the end of the workday, store all tools, equipment, and supplies and notify relief staff of all unusual occurrences.

11. Wash hands before and after performing any service for the resident.

12. Perform routine housekeeping duties that relate to nursing care procedures (i.e. cleaning tub room between uses.)

13. Wash hands before entering and leaving an isolation room or area.

14. Never leave the residential unit unattended.

14. Report any communicable or infectious disease that you contact to the Director of Nursing Services.

15. Follow established isolation and universal precautions and procedures.

 

Care Plan Functions

1. Review care plans weekly and when changes occur to determine if changes in the resident's daily care routine have been made on the care plan.

2. Inform the Charge or Staff Nurse of any changes in the resident's condition so that appropriate information can be entered on the resident's care plan.

 

Resident Rights

1. Maintain the confidentiality of all resident care information.

2. Ensure that you treat all residents fairly, and with kindness, dignity, and respect.

3. Ensure that all nursing care is provided in privacy.

4. Knock before entering the resident's room.

5. Report all grievances and complaints made by the resident to the Charge or Staff Nurse.

Qualifications:

Working Conditions and Qualifications

Sits, stands, stoops, bends, lifts and walks intermittently during the workday. Lifts at least 50 pounds. Responsible for transferring and turning residents. Be involved with residents, personnel, visitors, government agencies & personnel, etc. Be subject to hostile and emotionally upset residents, family members, etc. Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the department.

Education

Must possess, as a minimum, an 8th grade education.

 

Experience

Must be a Certified Nurse Aide in accordance with laws of the state of Illinois.
