Required qualifications: Director Care Management Registered Nurse
- The Director, Care Management provides strategic and operational leadership for all care-management functions across the acute-care ministry, including case management, utilization management, discharge planning, social work services, and psychosocial interventions that support safe, effective, and sustainable transitions of care.This leader is accountable for patient outcomes, satisfaction, throughput, and care continuum utilization while driving performance that meets or exceeds regional and national benchmarks. The Director develops and executes ministry-specific strategic and operational plans, manages key clinical, operational, and financial metrics, and ensures services remain patient-centered and aligned with the Triple Aim.
Qualifications: Registered Nurse Director Care Management
- Bachelor's degree in nursing AND a master's degree in a health-related/science field. Experience 2 years of care management experience in an acute-care setting.
- 2 years of healthcare leadership or management experience related to acute-care case management.
- California Registered Nurse (RN) License
- Strong knowledge of acute-care case management principles and practices.
- Understanding of evidence-based practice and care-transition models.
- Knowledge of the care continuum and effective transition-of-care strategies.
Pay: Up to $333,000.00 per year
