Essential Duties and Responsibilities
In this role you will be:
- An active member of the interdisciplinary treatment team and work collaboratively in the development, implementation, and revision of each patient’s interdisciplinary treatment plan.
- Observing and evaluating each patient as well as documenting their observations and evaluations.
- Providing the RN with relevant clinical information.
- Using nursing knowledge of medical, surgical, and behavioral health nursing to effectively provide direct patient care, observing, and documenting outcomes
- Providing patient and family education.
- Working collaboratively as a member of the interdisciplinary team.

QUALIFICATIONS
Required:
- EDUCATION: Graduate of an accredited LPN program
- EXPERIENCE: 1 year of medical surgical experience preferred
- LICENSURE/CERTIFICATION:
- BLS required or obtained within 1 month of hire
- Current license as a Licensed Practical Nurse in Pennsylvania
