Schedule: Available shifts include but not limited to (A) Tuesday through Saturday (B) Sunday through Thursday (C) Weekdays with rotating Saturday(s)

Position Summary

We are seeking a knowledgeable and compassionate Long Term Care Pharmacist to join our team. The ideal candidate will oversee medication dispensing, perform clinical reviews, collaborate with healthcare providers, and ensure compliance with state, federal, and facility regulations. This role is critical in supporting the unique medication needs of elderly and medically complex patients in long-term care settings.

Key Responsibilities

• Review and verify medication orders for accuracy, appropriateness, and regulatory compliance.
• Evaluate patient medication profiles for drug interactions, contraindications, allergies, and therapeutic duplications.
• Provide clinical consultation to physicians, nurses, and facility staff regarding medication therapy and best practices.
• Ensure accurate dispensing and timely delivery of medications to long-term care facilities.
• Maintain compliance with federal and state pharmacy regulations, Medicare, Medicaid, and long-term care requirements.
• Supervise and provide guidance to pharmacy technicians and support staff.
• Assist with formulary management, medication shortage mitigation, and emergency medication supplies.
• Pharmacist Verification 1 and Pharmacist Verification 2
• Participate in on-call and after-hours pharmacist coverage as required.

Qualifications

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited institution.
• Active pharmacist license in good standing in the state of practice (New York State license required)
• Long-term care, institutional, hospital, or consultant pharmacist experience preferred.
• Strong knowledge of geriatric pharmacotherapy and medication management.
• Familiarity with long-term care regulations and pharmacy management systems.
• Excellent communication, organizational, and problem-solving skills.
• Ability to work effectively in a fast-paced, team-oriented environment.

Preferred Qualifications

• Experience with QS/1 and DocuTrack
• Knowledge of Medicare Part D, Medicaid, and CMS regulations.
• Knowledge of New York State pharmacy establishment laws and regulations
