The Clinical Manager plays an important role in overseeing day-to-day clinical operations, supporting field staff, coordinating care, and ensuring strong communication between patients, families, referral sources, and the care team. This position also works closely with agency leadership to support quality initiatives, operational efficiency, and team development.

Responsibilities:
- Provide clinical leadership and operational oversight for agency services including nursing, therapy, home health aide, and direct care staff.
- Support the coordination of patient care services from referral through admission and ongoing service delivery.
- Review referrals and clinical information to ensure services are appropriate and aligned with agency capabilities and payer requirements.
- Assist with intake, scheduling coordination, and care planning to ensure timely initiation of services.
- Supervise, mentor, and support clinical staff while promoting a culture of accountability, professionalism, and teamwork.
- Ensure clinical documentation meets agency standards as well as state, federal, and payer regulatory requirements.
- Collaborate with physicians, hospitals, referral partners, and internal teams to ensure continuity of care and positive patient outcomes.
- Assist in resolving patient concerns, care coordination challenges, and clinical questions that arise during service delivery.
- Monitor quality indicators and support compliance with home health regulations and agency policies.
- Participate and lead staff training, competency development, and ongoing professional development initiatives.
- Support quality improvement initiatives, compliance audits, and operational planning.
- Promote a culture of collaboration, respect, and continuous improvement across the clinical team.

Qualifications:
Qualifications:
- Current RN license in the State of Iowa (or compact state).
- 3–5 years of experience in home health, hospice, or a related post-acute clinical setting.
- Prior experience in intake, case management, or home health leadership strongly preferred.
- In-depth knowledge of Medicare Conditions of Participation, OASIS, payer requirements, and home health documentation standards.
- Proven ability to lead and mentor teams while maintaining a high standard of clinical accuracy and efficiency.
- Strong interpersonal and communication skills with the ability to build rapport across multiple departments and external partners.
- Proficiency in EHR systems
- Demonstrated success in process improvement, workflow design, and performance monitoring.
Salary : $65,000 - $80,000
