Position Summary
Support patients with diabetes in Iowa by helping them access the full range of services offered by the Iowa Diabetes team, including clinical care, education, technology and research. This role emphasizes delivering high-quality care and education to improve patient outcomes. Serves as an advocate for patients, guiding them in overcoming challenges related to diabetes management and supporting their journey toward better health. Support and guide clinicians in optimizing their time and improving efficiency.
Core Job Responsibilities
This job description outlines the primary responsibilities of the position; however, the employee may be required to perform additional duties as assigned by the supervisor or company leadership.
Respond promptly to inquiries from prospective patients regarding our services and assist them in navigating and accessing IDC and IDU services tailored to their individual needs. Contact patients to confirm readiness for both new and follow-up appointments, and provide education to current and prospective patients on the benefits and use of the patient portal. Greet and room patients in a welcoming and professional manner, obtain vital signs and A1Cs, and assist with clinical procedures as directed by the clinician on duty. Additionally, manage patient check-in and check-out processes for both in-person and virtual visits, ensuring that the billing process is completed accurately. Be a scribe in the patient room. At the direction of providers, coordinate laboratory services by ordering tests, drawing blood, and monitoring results. Additionally, provide procedural support as needed by the attending clinician.
Provide diabetes-related education to patients as part of the Diabetes Education Program. Complete required diabetes education training and deliver services under the guidance and supervision of delegated personnel. Conduct individual education sessions, offered in person or virtually, and support the achievement of weekly and quarterly education goals. Follow up with patients who request additional information or resources.
Minimum Knowledge, Skills and Abilities Required
- Prior experience in care coordination, case management, patient navigation, or community health strongly preferred
- Preferred PCT, CNA, CMA, MA, LPN, RN
- At least 1 year of work experience
- Knowledge of business English, spelling, arithmetic, modern office practices and procedures, standard office equipment
- Proficiency in Microsoft Office/Google Suite of products
- Understanding of diabetes and medical terminology
- Ability to manage multiple priorities in a fast-paced environment
- Patient-centered mindset and commitment to health equity
