What are the responsibilities and job description for the Patient Care Navigator Level I position at Iowa Diabetes?
Position Summary
Help patients with diabetes in Iowa navigate the value Iowa Diabetes team can provide them through research, care, education and technology. Become a champion for the patients and help them solve their diabetes problems and have better health. The working hours for the position will be determined upon hire. The candidate should have the ability to support evening phone calls on a routine basis and occasionally assist with weekend calls/outreach events as well.
Core Job Responsibilities
This is the expected job description but the employee may be asked to perform other duties assigned by the Supervisor or company leadership.
Actively seek patients for enrolling studies using Patient Qualifier and by screening medical records for subsites and partner clinics, identifying potential patients using inclusion/exclusion criteria. Work with the Clinical Research Team to adequately recruit patients based on the criteria provided and schedule screening visits.
Minimum Knowledge, Skills and Abilities Required
- Associates degree and/or 3 years of work experience in a customer-facing role
- Professional and friendly communication style that builds rapport quickly in-person and via phone
- Demonstrated ability to analyze data from multiple sources to make decisions
- Ability to document information accurately and concisely
- Proficiency in Microsoft Office/Google Suite of products
Preferred Knowledge, Skills and Abilities
- Bachelor’s degree
- Research experience
- Understanding of medical terminology
- Outbound calling experience
