We are seeking an experienced Hospital Liaison to support referral development and admissions in the Schaumberg, IL area. This is a hospital-based position, requiring regular travel to assigned hospital accounts rather than working from a single facility.

- Develop and maintain relationships with hospital case managers, social workers, discharge planners, and other referral sources.
- Meet with patients and families to explain skilled nursing and rehabilitation services and assist with the transition of care.
- Coordinate referral reviews with Central Intake and facility teams to determine clinical and financial eligibility.
- Follow referrals through admission, communicate updates, and help resolve admission barriers.
- Maintain accurate, timely documentation in ExaCare.
- Support referral growth and meet established admission goals.
- Previous experience as a Hospital Liaison or in a comparable skilled nursing admissions or healthcare business development role.
- Knowledge of hospital discharge planning, skilled nursing services, and Medicare, Medicaid, and managed care payer requirements.
- Strong communication, relationship-building, organizational, and follow-through skills.
- Ability to work independently and manage multiple hospital accounts.
- Valid driver’s license and ability to travel regularly throughout the assigned territory.
