We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for our Long-Term Care (LTC) facility. This PRN position requires a skilled professional who is committed to providing high-quality patient care in various settings, including assisted living and nursing homes. The ideal candidate will have experience in critical care, geriatrics, and memory care, along with a strong understanding of medical terminology and patient assessment.
Duties
- Provide direct patient care, including administering medications and treatments as prescribed.
- Conduct thorough patient assessments and document findings accurately in electronic health record (EHR) systems.
- Collaborate with healthcare team members to develop and implement individualized care plans.
- Monitor vital signs and report any changes in patient condition to the supervising nurse or physician.
- Assist with IV infusion, catheterization, tube feeding, and other medical procedures as needed.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
- Educate patients and families regarding health management, medication administration, and care plans.
- Participate in discharge planning and case management for patients transitioning from acute to post-acute care settings.
Experience
Candidates should possess the following qualifications:
- Previous experience in long-term care, assisted living, or a similar healthcare environment is preferred.
- Familiarity with Epic or other EMR systems is a plus.
- Experience working with individuals with developmental disabilities, Alzheimer’s disease, or dementia is highly desirable.
- Critical care experience in settings such as ICU, ER, or Level I/II trauma centers is beneficial.
- Knowledge of ICD coding (ICD-9/ICD-10), phlebotomy, medication administration, and basic life support techniques.
- Strong communication skills and the ability to work effectively within a multidisciplinary team.
