We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for our Long-Term Care (LTC) facility. This PRN position offers the opportunity to provide high-quality patient care in a supportive environment. The ideal candidate will have experience in various healthcare settings, including long-term care, assisted living, and critical care, and will be skilled in patient assessment and medication administration.
Duties
- Provide direct patient care to residents, ensuring their comfort and safety.
- Administer medications and treatments as prescribed by healthcare providers.
- Conduct patient assessments and monitor vital signs.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Assist with IV infusions, catheterization, tube feeding, and other medical procedures.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Educate patients and families on health management and disease prevention.
- Participate in discharge planning and case management as needed.
- Support residents with activities of daily living (ADLs) while promoting independence.
Experience
Candidates should possess the following experience or skills:
- Long-term care experience, including nursing home or assisted living settings.
- Critical care experience within a Level I or Level II trauma center.
- Familiarity with medical office operations, including EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
- Experience in geriatrics, memory care, Alzheimer's care, dementia care, hospice care, or home health.
- Proficiency in administering injections, phlebotomy, venipuncture, and performing physical examinations.
- Knowledge of ICD coding (ICD-9 and ICD-10), medical terminology, anatomy knowledge, physiology knowledge, and infection control practices.
- Experience working with individuals with developmental disabilities or those requiring specialized care (e.g., ventilator management).
