What are the responsibilities and job description for the Support Coordinator position at Intersect Services, LLC?
We are looking for a worker in the Salt Lake, Ut area: Employee will need to come to Clearfield office at least 3 days a week for at least 3-6 months depending on grasp of job.
Bachelor's degree in a Human Services related field. Some Experience with individuals with intellectual disabilities and brain injury preferred. Must have own transportation, valid Utah Driver's license, and car insurance.
Support Coordinators assist individuals with disabilities and their families to develop plans to find the most appropriate services and select the most appropriate service delivery model, based on the individual person’s needs and wishes. The Support Coordinator is the first and best option to help a family determine what services are available to best suit a person’s needs. It involves working closely with individuals with intellectual disabilities and/or brain injury and their families; providers of services to people with disabilities as contracted with the Department of Human Services (UTAH) Division of Services for People with Disabilities (DSPD); and DSPD. Job duties include face to face meetings, assessments, reporting, team leading, and development of services to best meet and individual's needs.
Must demonstrate efficient writing skills using computer programs, problem solving, team leadership skills, able to work effectively, manage one's own time, become familiar with social services resources, and be a self-starter.
This is a very flexible schedule: some evenings and weekend work may be needed.
This job can be worked from office or home once proficiency with the job is met.
