BASIC FUNCTION

The Personal Care Coordinator is responsible for managing the day-to-day operations of LifeSpan Companion Care and supervising the staff of Personal Services.

RESPONSIBLE TO: LifeSpan Companion Care Manager

EDUCATION, QUALIFICATIONS, AND CREDENTIALS

College degree in human services field preferred and/or appropriate coursework. Direct experience working with seniors in a home health setting, senior housing situation or other relevant senior setting is required. Supervisory experience is required.

ESSENTIAL JOB FUNCTIONS

The employee must be able to perform each essential function effectively to be successful in this position.

1. Assures compliance with regulatory aspects of the Personal Services Program.
2. Reviews, proposes and implements policies and procedures that improve operations to promote increase quality services with annual budget guidelines.
3. Educates residents and family members on services available and promotes the availability of services.
4. Attains revenue goals for Personal Services as defined in the yearly budget.
5. Marketing-Health and Living in-house.
6. Training-Utilizes Relias Computer System and assures that caregivers receive proper training and in-servicing.
7. Evaluates service effectiveness/deficiencies and customer satisfaction; proposes and implements corrective action.
8. Promptly processes service calls and effectively establishes priorities for service delivery.
9. Recruits, trains and schedules caregivers. Selects staff for work assignments.
10. Responsible for maintaining accurate up-to-date resident admission packages, service plans and staff schedules, and shares appropriate information with the staff to ensure smooth operation of services at all times.
11. Adequately documents procedures and maintains an organized approach in handling routine operational matters.
12. Accurately prepares billing information and submits information in a timely manner.
13. Completes employee performance appraisals for designated staff in a timely manner. Demonstrates management skills in evaluating performance and setting goals with the employees supervised.
14. Assures designated staff complies with all policies and procedures.
15. Handles personnel matters in prompt and professional fashion including disciplinary action and/or terminations.

KNOWLEDGE, ABILITIES, AND SKILLS

Knowledge of current federal and state laws and regulations that apply to Personal Services. Actively listens, follows appropriate channels of communication. Ability to effectively present information, communicate and respond to questions from groups of managers, clients, staff and the general public. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability and emotional stability to deal with upset residents for families.

SPECIAL REQUIREMENTS

As determined in conjunction with LifeSpan Companion Care Manager, may attend seminars, workshops, and conferences to remain current on regulations and abreast of development in personal services care.
