Responsibilities of an In Home Licensed Practical Nurse (LPN)
- Provide compassionate in-home nursing care and assistance with daily activities.
- Monitor and report changes in patient condition to the RN Supervisor.
- Administer PEG tube medications & feedings as required.
- Assist with bathing, dressing, feeding, and mobility support.
- Perform general housekeeping to maintain a safe environment.
- Offer companion care to enhance client well-being.
- Coordinate with families and healthcare teams, including hospice services.
- Follow MD treatment plans and ensure compliance with patient care protocols.
Requirements for Home Health Licensed Practical Nurses
- Graduate of a practical (vocational) nursing program for LPN/LVN.
- Current license in good standing as an LPN in the state of South Carolina.
- Comfortable working in an in-home setting.
- Excellent verbal and written communication skills.
- Must be able to work independently.
- Reliable transportation and valid drivers license (if required to drive).
- Proof of auto liability insurance (if applicable).
- Must be able to pass a pre-employment background and drug screen.
- Able to stand, walk, bend, kneel, and assist patients with mobility (occasional lifting up to 50 lbs.).
