What You’ll Do as a Hospice RN Case Manager:
- Complete holistic assessments addressing physical, emotional, psychosocial, and spiritual needs
- Develop individualized care plans that honor each patient’s goals
- Collaborate with an interdisciplinary team including physicians, social workers, chaplains, CNAs, and volunteers
- Provide education to patients and families on symptom management, medications, and the hospice journey
- Deliver compassionate end-of-life care and comfort measures
- Maintain accurate documentation in accordance with organizational and regulatory standards
- Participate in quality initiatives that elevate hospice care for the community
What We’re Looking For:
- Graduate of an accredited nursing program with an active Louisiana RN license
- At least 1 year of hospice nursing experience
- Current CPR certification
- Valid driver’s license, auto insurance, and reliable transportation
- Strong clinical assessment skills with a compassionate, patient-centered approach
- Knowledge of state and federal hospice/home health regulations
